TCARE’s process begins with the family caregiver completing a 32-question Family Caregiver Assessment.
The TCARE system generates an Assessment Summary Sheet that provides a profile of the family caregiver’s needs and strengths.
The TCARE proprietary decision algorithms identify optimal treatment goals, support strategies and a prioritized list of services types tailored to the family caregiver’s needs. The care manager then uses the system to locate resources available within the family caregiver’s community.
The care manager develops a Care Plan Consultation Worksheet that includes specific service options available to the family caregiver and are in line with previously identified goals and support strategies.
Family caregiver and care manager discuss outcomes from the assessment process and the potential benefits of services and resources that are being recommended before jointly identifying services to be included in a care plan.
The care manager writes a care plan that reflects decisions made during the consultation and is specifically designed for the family caregiver. The care plan includes detailed service information–spanning start/end dates, specific family caregiver/care manager actions and a follow-up plan.
